Halton Healthcare's Better Care Story

What if transitioning between hospitals and long-term care homes could be seamless and safer? Halton Healthcare has joined Project AMPLIFI, an innovative initiative aimed at achieving just that.

Project AMPLIFI improves health outcomes for patients by using an integrated care coordination platform. This reduces medication errors, diagnostic delays, treatment holdups, and readmissions for high-risk populations.

AMPLIFI stands for Allergies, Medications, Problem List, and Immunizations For Integration.

At each transfer point, patient visit information is transmitted into the electronic health record of the receiving facility. This shared information includes encounter details, provider notes, lab and imaging results, medications, discharge summaries, and more.

Deanna Mol, Interim Director Digital Health Optimization at Halton Healthcare, shares her thoughts on the initiative:

This is a good opportunity to uphold patient safety and improve the patient experience. Patients don’t want to have to tell the same story over and over again, and this gives us the opportunity to do more confirmation. This is helping us streamline a lot of that work prior to them being with us.

Deanna continues, “It’s really encouraging that this project isn’t just about hospitals and that we’re looking at the bigger continuum of care, rather than just acute care to acute care, which is bringing in the conversation that we are one province providing health care. I’m proud of our team and their hard work and collaboration with our partners in the AMPLIFI team and Meditech.”

This comprehensive data provides caregivers with a full understanding of the patient’s journey, minimizing the need to gather information from external sources. It doesn’t rely on the patient’s memory, thus reducing errors and offering crucial details in emergencies.
