Halton Healthcare’s Halton@Home Program reaches more patients than ever before.

When you leave the hospital, recovery doesn’t stop at the door. For many patients, the days and weeks that follow a discharge are when support matters most. That’s the thinking behind Halton@Home, a Halton Healthcare program that brings short-term, clinically supported care directly into patients’ homes.

Launched in March 2024, Halton@Home has already supported more than 1,000 patients recovering from Oakville Trafalgar Memorial Hospital (OTMH), Milton District Hospital, and Georgetown Hospital. Between December 2025 and February 2026, the program reached a record 286 patients in just three months.

The program is designed for patients who are medically stable after discharge but would benefit from continued care as they settle back into daily life. An integrated team of nurses, respiratory therapists, rehabilitation professionals, and personal support workers delivers tailored care based on each patient’s individual goals, needs, and home environment.

A new pathway for patients who need to breathe easier

In January 2026, Halton@Home added a new Respiratory Pathway to support patients with mild to moderate breathing difficulties who may require short-term oxygen. Previously, many of these patients would have remained in hospital while their oxygen levels stabilized. Now, eligible patients can recover at home with close clinical monitoring and around-the-clock support if their condition changes.

The impact shows up in everyday moments. One family member of a patient in the program shared:

The quality of care has been excellent, and all the staff have been great and so helpful. The program has been a huge help. I’m nearly 80 myself and this program helps in areas I need the most, which is showering my mom and looking after her wellness.

By supporting patients at home, Halton@Home also helps ease pressures across the broader health system. The program supports timely discharge, reduces avoidable emergency visits, and frees up hospital beds for patients who need them most.

It’s a model of care that meets people where they are and follows them all the way home.
