Helping Young Adults Take Their Next Step

With help from generous donors, Oakville Trafalgar Memorial Hospital (OTMH) launched the Transitional Aged Youth (TAY) Program to support young adults going through a difficult stage of life.
The program is designed for people aged 18 to 25 who are facing mental health challenges, life changes, and recovery after the COVID-19 pandemic. Many are dealing with disrupted education, isolation, and stress about their future. The TAY Program helps fill a long-standing gap between youth and adult mental health care.
TAY meets young people where they are – on campus, in a coffee shop, or out in the community. It offers one-on-one support, counselling, and help with setting goals and building structure. Participants also learn important life skills like writing a résumé, sending emails, or making phone calls.
More than 30 young people have already taken part in the program. They are rebuilding their confidence, getting involved in their communities again, and moving forward with more support.
Thanks to donor support, these young adults are finding their strengths and building brighter futures.
