Through home visits, remote monitoring, and telehealth, Halton @Home provides seamless, personalized healthcare, helping patients transition safely back into their daily lives—reducing gaps in care and lowering the risk of readmission.

As Halton Region experiences rapid growth—with its population expected to rise by 60% over the next 25 years, including a sharp increase in seniors—the demand for acute healthcare services is escalating. This shift adds new layers of complexity to patient care and increases pressure on hospitals and healthcare providers.
